Cavity Prevention Plan

A prevention-first promise to eliminate unnecessary fillings

The Premise

What if you could reverse tooth decay before it progresses further? It’s possible with our Cavity Prevention Plan utilizing Curodont™ Repair Fluoride Plus (the “Prevention Plan”)

Curodont™ Repair Fluoride Plus (“Curodont™”) is a non-invasive, drill-free, pain-free regenerative treatment (hydroxyapatite generation) that can re-mineralize tooth enamel. Curodont™ is designed to reverse and/or arrest early-stage cavities (incipient lesions) and white spot lesions. In a recent test involving 42 North Dental practices, 96% of teeth that received minimally invasive treatments like Curodont™ did not require a subsequent filling or another restorative procedure on the same tooth surface within the following 24 months. Please talk with your dental provider to learn more about Curodont™.

The Prevention Plan

Your Prevention Plan begins when your dentist diagnoses an “early” cavity (incipient lesion) or white spot lesion and treats the affected tooth surface with Curodont™. With proper follow-up care, we expect the treated tooth surface to stay cavity-free for two years. To qualify for and maintain your Prevention Plan, you’ll need to maintain your exams, x-rays, and hygiene appointments for the next 24 months.

If the treated tooth surface does develop a cavity within 24 months of treatment with Curodont™, we’ll treat it with a conventional filling (restoration) at NO cost to you.

Your participation in the Prevention Plan is entirely voluntary, and you may terminate this participation at any time.  Any refusal to receive Curodont™ treatment(s) and/or participate in the Prevention Plan will not, in any way, impact the services or treatments otherwise available to you from your dental provider.

Note that for clinical documentation and treatment planning, teeth are described using five surfaces. Posterior teeth have an occlusal surface, Anterior teeth have an incisal surface, while all teeth have mesial and distal proximal surfaces, a facial surface (buccal on posteriors, labial on anteriors), and a lingual surface (lower teeth) or palatal surface (upper teeth).  These surface designations allow dental providers to specify the exact extent and location of caries or restorative treatment. The plan covers treatment per surface as documented clinically.

How to Qualify:

To stay covered under the Prevention Plan, you must:

  1. Treat early cavities with Curodont™

If your dentist identifies an early cavity (incipient lesion) that’s eligible for treatment with Curodont™, with your informed general consent, your dental provider will treat it non-invasively with Curodont™ to help stop it from progressing.

  1. Maintain all preventive appointments as recommended by your dental provider for 24 months
  • Annual clinical exam with your dentist (once every 12 months)
  • Hygiene appointments (i.e. cleanings) as recommended by your dental provider (usually every 6 months, minimum of 2 per year)
  • Annual x-rays (radiographs), as recommended by your dental provider (minimum of once every 12 months)
  1. Follow recommended home care

Brushing, flossing, and fluoride use as directed.

What’s Covered

If the surface of the tooth treated with Curodont™ develops a cavity within 24 months of the date of application:

  • Your dental provider will provide a conventional filling (restoration) at no cost to you provided all conditions of the prevention plan have been met (see “How You Qualify” section above).

What’s Not Covered

  • Any tooth surface not treated with Curodont™
  • Cavities that have developed on any tooth surface treated with Curodont™ if exam, x-rays, or hygiene appointments are missed or cancelled
  • Restoration needed due to trauma, fractures, or other non-decay-related causes

Full Disclosure

The Prevention Plan is a limited time offer available only at 42 North Dental supported practices and is subject to all applicable federal, state, and local laws and regulations. Eligibility is limited to tooth surfaces diagnosed by a 42 North Dental supported dentist as having an incipient lesion or white spot lesion appropriate for treatment with Curodont™.  Such dentists shall exercise their independent professional judgment when diagnosing lesions and developing and implementing treatment plans. Curodont™ treatments furnished by dentists outside of the 42 North Dental network are not eligible for coverage under the Prevention Plan.

The lesion must be identified, treated with Curodont™, and the patient’s financial responsibility paid on the same date of service. Activation of the Prevention Plan requires compliance with all dentist-recommended preventive care, including maintaining appointments for examinations, hygiene visits, diagnostic imaging, and home care.

The Prevention Plan applies only to the specific area of the tooth treated with Curodont™ and is void if appointments are missed, care is not followed, or if restorative needs arise from trauma, fracture, or non-decay causes. If all conditions are met and the treated tooth surface develops a cavitated lesion within 24 months of application, a single surface conventional filling (restoration) will be provided at no cost to the patient. This promotion has no cash value, is non-transferable, and may be modified or discontinued as permitted by law.
